You need to take him to a vet as it sounds serious. Is it possible someone put an elastic band around his neck that has become embedded in his skin? Whatever the cause is you need to get him checked out by a vet to figure out what the cause is and get him the treatment he needs.
Sassy is right. It could be anything from allergies to fleas to ringworm to what Sassy said. We can't give out free medical advice here because we aren't vets, just people who love kitties. You need professional medical advice and treatment from someone who is qualified to do so. If you can't afford it, many shelters and rescues know of vets who will treat at a reduced cost.
